Absolutely not. If they really, really want that info they can get it looking up your phone or email address online. Work info is a non-starter for me also.

I'm not sure most of this is right? She does send you to her form, but as far as I know she will answer very direct questions about booking (time/location) over email. It's possible she has gotten very restrictive about the work thing, but she was not when she started a few years ago.

I am very affirmative. She now asks ID and you have to fill form and send deposit with no possibility of direct questions by e.mail before booking.
