If you're still stuck, here are some ideas.  Even if you don't use any of them, maybe one of them will give you a spark of inspiration. 

- You could change the point of view to the doctor's. 

- We could see the doctor demonstrating her physical and mental prowess in athletic and mental competitions.

- You could shift to the doctor training her niece to build muscle and become a dom.
